I would suspect the problem is the actual mkv file though. It is configured with some intelligence where it can send the file to different encoders depending on what the properties of the source file are based on things like resolution, audio layout, container format, etc., but once I had that configured, I almost never had to touch handbrake again. This can be set up to also make a lot of different formats, but is a bit more complicated to configure than handbrake and xmedia recode and requires some testing and tweaking of the analyze and encode settings to get set up. I also have FFAStrans installed for doing transcodes.
I know it is extra conversion time but it does make the editing a lot easier. The quality is about the same as a Grass Valley HQX file.
Prores 422 or 422 HQ is a great codec for editing because it is an I-Frame codec and not a long GOP codec like most H264 and H265 files are. Once the MP4 is made, I will often take that ad run it through xmedia recode to make a prores file from it. When I have encountered this before I have had to resort to using Handbrake to make an MP4 file as it seems to be more forgiving with files that have problems, but made it at a high bitrate so as not to degrade the quality any further and made sure the audio bitrate was also high, as handbrake defaults to a low bitrate. I have occasionally encountered a crash before if the mkv is not made correctly.
